All students can have up to double the allotted time for exams if requested.
Students requesting to write their exam in a more isolated space must provide documentation from a professional (e.g. IPP, IEP, Educational Assessment, etc).
Does this student have any physical, intellectual, behavioral or emotional needs which would impact our ability to provide an appropriate educational program?
Has this student been provided with an Individual Program Plan (IPP) or Individual Education Plan (IEP)?
Please provide details of any testing completed, special needs requirements: including learning needs, special education programs, IPP/IEP, etc., at the student's previous school or at the recommendation of an educational specialist/psychologist, as well as any additional exam accommodation or assistance required.
Are there any other factors (disabilities, illnesses, location, family concerns, etc.) that may affect the student's progress in their studies?
